Lower blepharoplasty case: under-eye fat repositioning and micro fat grafting for a woman in her 50s seeking smoother, younger-looking eyes.

My under-eye wrinkles are so fine-lined, and I look hollow and tired — it makes me look so much older. ㅠ

1. Lower eyelid: The skin beneath your eyes has lost its elasticity and appears to sag. There is puffiness from under-eye fat along with a pronounced tear trough deformity.
2. Upper eyelid: The strength of your eye-opening muscles differs between sides and is generally weak, resulting in multiple fold creases and a hollowed appearance of the upper lid. There is also skin laxity present, so upper blepharoplasty (including ptosis correction) would be recommended. (She said she would consider it after seeing how the lower blepharoplasty turns out. ^^;)

We performed under-eye fat repositioning along with micro fat grafting. The sagging skin was, of course, resected as well.
What do you think? Does she look noticeably smoother and younger? ^^; She mentioned she would come back for upper eyelid surgery once the weather gets warmer — and she even brought three whole boxes of dried persimmons. We will enjoy them gratefully!
* Key Points
1. Attempting to correct sagging under-eye skin by removing too much of it can cause ectropion (lower eyelid eversion). This not only looks aesthetically poor but also causes significant eye irritation and discomfort for the patient. Aggressively over-resecting skin is never the answer — it can lead to serious complications!
2. When micro fat grafting is performed simultaneously, patients also benefit from improvement in dull, uneven skin tone. Choosing the right candidates for this combination approach yields truly excellent results.
